How To Refer a Service User

If you would like to make a referral into any of our residential/ nursing care homes, call us FREE on 0800 313 4774 or complete the form below.

How the referral process works:

  1. Initial enquiries are sent to our Head Office
  2. The Head Office will then send your contact details to the relevant service
  3. The Operations Manager or Home Manager will contact the referrer to discuss the potential resident
  4. An assessment will be carried out and if the resident is suitable for the service, an offer letter will be issued confirming placement offer and the costs of placement
  5. If the offer letter is accepted, a planned admission date will be agreed with the mental health service and referrer
  6. A license agreement will then be issued, and this should be signed, dated and returned by the referrer to the mental health service prior to the commencement of the placement.
